What does a care manager actually do?

A care manager is a Registered Nurse or Licenced Medical Social Worker who serves as you or your loved one’s dedicated advocate and care coordinator. Our Care Managers provide personalized consultation, advocacy, and support tailored to each client’s unique needs and circumstances. Services begin with a comprehensive assessment that evaluates the client’s current condition, medical needs, social and emotional dynamics, personal preferences, support systems, and overall safety. From there, we provide thoughtful recommendations and guidance that may include, but are not limited to:
